To apply for a place on the 2012 Llysfasi Spirituality Workshop, you will need to obtain an application form and two forms for references. You can download and print them using the links on this page, or you can request them from Mary Rose Fitzsimmons HHS (Llysfasi Co‑ordinator) at the e‑mail address below:

Please print out, complete and send along with any supporting material by post to Mary Rose Fitzsimmons HHS (Llysfasi Co-ordinator) at the address provided.
DOWNLOAD REFERENCE FORM
Please print out and send to your two referees for completion. You could alternatively e‑mail it to your referees for them to print out themselves.

The closing date for applications is November 30th 2011. Enquiries after this date could be worthwhile as there are sometimes cancellations, but in general early application is advisable. Your application form and references are treated in the strictest confidence.
