To apply for a place on the 2009 Llysfasi Spirituality Workshop, you will need to obtain an application form and two forms for references. Please print out, complete and send along with any supporting material by post to Mary Rose Fitzsimmons HHS (Llysfasi Co‑ordinator) at the address provided.
DOWNLOAD REFERENCE FORM
Please print out and send to your two referees for completion. You could alternatively e‑mail it to your referees for them to print out themselves.

The closing date for applications is 1 June 2009. Enquiries after this date could be worthwhile as there are sometimes cancellations, but in general early application is advisable. Your application form and references are treated in the strictest confidence.
